What am I expected to bring at my office visit?

Bring your medical insurance card to your appointment. It is also helpful to bring a current list of medications that you are taking. Please plan to arrive 10 to 15 minutes prior to your scheduled appointment time. This will ensure we have up to date information and paper work is completed.

How can I renew my prescription?

Please contact your pharmacy and ask them to send an electronic request. Allow 48 to 72 business hours for the request to be completed.

How can I obtain a copy of my medical records?

You will need to sign a release form in order for us to release your medical records. This form can be found in the patient forms tab under patient information.

What is the policy on canceling and rescheduling?

We understand that unforeseen circumstances may arise, and you may need to cancel or reschedule your appointment. To best serve all our patients, we request that you provide at least 24 hours notice for cancellations or rescheduling. Failure to do so may result in a $50 no-show fee.

Co-Payments and Deductibles

Co-payments and any applicable deductibles are due at the time of your visit. These fees are determined by your insurance plan and are your responsibility. Please be prepared to make these payments to avoid delays in your care.

Do you take walk-in appointment?

We usually have same day appointments or next day appointment available. Please call ahead before coming in to verify an appointment opening is available.

How can I message or email my doctor?

All patients will have access to create a patient portal where one can send and receive messages with their provider. If you haven’t already, please contact our office and we’d be happy to help you out.
